Life Insurance Protection for You and Your Family

Life Insurance is a highly effective way for PGA of America Members to help protect their loved ones. Optional Term Life Insurance is available for PGA of America Members to supplement their existing in-dues coverage issued by The Prudential Insurance Company of America (Prudential). The Optional Term Life insurance benefit is available to active PGA of America Members under age 65 with insurance up to $500,000 for members and $250,000 for spouses.

How to enroll: Complete ALL forms below and return them to us via email at GGB.AffinityPHL@ajg.com. You can also contact us at 800-952-4050 with questions.

How to file a Claim: Please contact the PGA Membership Department at (800) 474-2776.

Coverage Begins: First of the month after approval of proof of good health.

Coverage Ends:

Coverage ends the date group contract or plan is canceled, date the member is no longer a member of a covered class, date covered class is no longer covered, last day of period member made required contributions, or last day of active membership.

The above coverage applies to those under 65. At age 65 to 69, value is reduced by 35% of original face value (of age 64 amount) and at age 70 to 74, value is reduced to 50% of original face value (of age 64 amount). Coverage ends at age 75.*

Assistance for beneficiaries: Prudential provides members and their families with help during the very difficult period that follows the death of a loved one. The Beneficiary Resources offers access to emotional support as well as guidance on how to plan a funeral or file a death benefit claim.**
