DHMO – Dental Health Maintenance

  • Organization offering no claim forms no deductibles, no maximum annual dollar limits and no waiting periods. Services received from an in-network provider with scheduled co-payments from the subscriber.

PPO – Preferred Provider Organization

  • Offering the flexibility to select a participating in-network dentist for lower out-of-pocket costs and no claim forms or use any licensed dentist (out-of-network). Claim forms required for out-of-network providers and reimbursement is based upon the insurance carriers maximum allowable charges.
